Gestational age counts forwards from the conventional starting point and is the unit clinical care uses: appointments, screening windows, growth assessment and viability thresholds are all expressed in completed weeks and days. A countdown counts backwards from the estimated due date and is the unit planning uses: leave dates, preparation, travel decisions.

Trimester boundaries are conventions

Trimester boundaries are not fixed physiological events. The most common convention places the first trimester through 13 weeks 6 days, the second from 14 weeks to 27 weeks 6 days, and the third from 28 weeks onwards — but you will find sources that draw the lines slightly differently.

The clinical decisions that matter are tied to specific gestational ages, not to trimester labels. Screening windows, for example, are defined in weeks and days with a few days of tolerance either side. Use the exact gestational age for anything that has a date attached; use trimester language for general description.

What a countdown does not mean

A countdown implies an appointment being kept, which pregnancy does not guarantee. Term is a range, not a date: births at 37 to 42 weeks are all within normal limits, and only a small minority occur on the estimated due date itself.

This matters practically. Plans built to end exactly on the due date have no slack in either direction. Where a decision depends on the birth having happened — handover, cover, travel — build in tolerance on both sides rather than treating the countdown as a schedule.

When the due date changes

Estimated due dates are revised, usually after a dating scan and occasionally later. When that happens, every derived figure moves with it: gestational age, days remaining, trimester position and any milestone dates you had recorded.

Recalculate from the current date rather than adjusting old figures by hand. Adjusting one number and forgetting another is how a set of notes ends up internally inconsistent, with no obvious sign which entry is stale.
